Windows Icon is displayed too small (ICO-Format)

I have created my own icon in the format ICO. The icon consists of multiple layers, 16x16 pixel, 32x32, 48x48, 64x64 and 128x128.
Nevertheless, the icon is maximal shown in a size of 48x48 pixels in the Windows Explorer. Even if I set the view to maximum, the icon remains in that litle size. Surprisingly, the 16x16 image size is working in the smaller list view.
The problem is the same when applying the icon to an application or a folder. What am I doing wrong here? What can I do to make Windows accept the larger sizes?

Windows is using the 256x256 icon for all views that require larger icon than 48x48 pixels. For smaller views such as 128x128, the 256x256 icon is scaled down.
Add another layer containing a 256x256 image to your icon, then it should work.
