Usually, a penalty shootout is continued until a winner has been clearly determined. Theoretically, this can take forever if both teams are shooting the same number of goals and the goalkeeper is holding the same number of penalty kicks.

The longest penalty shootout, I have ever heard about, was in a match between two football clubs in Hamburg. It happened in the first round of the 2. Liga-Pokal 2008 between Eintracht Norderstedt II and SC Sternschanze II.
The game lasted 3 hours and 10 minutes. Eintracht Norderstedt II won with 21-20.
